Before you begin

You need to be in our system before creating an account. Policyholders may need to check with an HR representative or group admin about the policy start date.

Identification Number

If you do not know the identification number, use the social security number of the policy holder and your date of birth.

ID Card Example, Your member ID can be found on your UCD insurance card
