You have the option to sign up to receive electronic documents or notices for any of the following dental contact programs. Before signing up, please read the section titled 'Important Information' and the Statement(s) below that are applicable to your communication preferences selection(s).
Once you have read the 'Important Information' section and any of the below sections ("Statement(s)") that apply to your communication preferences selection in which you have an interest, agree to these Member Communication Preferences Terms and Conditions and then submit.
The paperless process is complete only after successful notification of an electronic document and/or notice is delivered to the e-mail address you provided, as applicable.
To retrieve electronic documents, you must have: (1) access to the internet through a current browser; (2) an e-mail account; and (3) the Adobe Acrobat application to view the EOBs and any other document in PDF format.
We will use your preferences whenever we can. However, there may be times when we will have to send information to you another way.
Please Note: To help ensure receipt of e-mail and electronic documents, you may need to update your anti-spam protection filters to allow our e-mail notices to reach you, if applicable. If you experience difficulties receiving e-mails, you may contact Customer Service using the number on the back of your ID card to update your e-mail address if you changed your e-mail address or if you are unable to access or read your electronic documents. You can also change your e-mail address by logging into your account on your dental plan’s website as a member and updating your contact information. If we are unable to resolve issues with delivering electronic EOBs to you or relating to your ability to access or read electronic documents, your paperless selection of electronic notices will end.
If you wish to receive your Explanation of Benefits (“EOBs”) via electronic format, we will alert you each time an EOB is issued via the e-mail address you provided, as applicable.
When you sign up to receive electronic documents you will retrieve EOBs from your dental plan’s website instead of receiving these documents in paper through the U.S. mail.
This paperless option applies to EOBs issued in connection with claims that are submitted after services have been rendered as well as those issued after a predetermination request is made for your dental plan to estimate benefits for a dental treatment you have not yet received. Once EOBs are posted online, they will be available until 12 months from the original date of service or predetermination request for that particular claim (inclusive of any processing time).
You have the right to request and obtain a paper version of an EOB from Customer Service at no charge, even after they are no longer available online. To request a paper copy of your EOB, contact the number on the back of your ID card. You can also print these electronic documents from your dental plan’s website and/or save them to your computer.
Please Note: You can sign up to receive paper by logging on to your dental plan's website as a member and updating your contact information, then changing the checkmark from the desired selection in your communication preferences. You may also change your paperless EOB option by calling the Customer Service number on the back of your ID card. If your coverage is terminated or otherwise cancelled, you will receive notice of and have access to this website and your electronic documents for up to one year following the termination or cancellation date.

Members with select insurance plans may receive Member-Related E-mails. In addition to other valuable information, Member-Related E-mails will include information to help you understand your benefits, get the most out of innovative tools, and stay well with oral health and wellness tips for you and your family. When you sign up for Member-Related E-mails, you will receive periodic e-mails at the e-mail address you provided.
Please Note: If you choose not to receive Member-Related E-mails, you can change the checkmark from the desired selection in your communication preferences via the website or click the “Unsubscribe” link at the bottom of each Member-Related E-mail you receive.
To sign up, members must accept these Member Communication Preferences Terms & Conditions and then submit. If your coverage is terminated or otherwise cancelled, you will continue to receive Member-Related E-mails unless you click the “Unsubscribe” link found on the bottom of every e-mail you receive after your coverage has ended.
Members with select insurance plans may receive My Oral Health E-mails that include wellness education information focused on keeping you well through your oral health if: (1) you self-report a covered condition via the website or by contacting Customer Service; (2) your medical data identified a covered condition; or (3) your dental claims identified services related to a covered condition. When you are signed up for My Oral Health E-mails, you will receive periodic e-mails at your provided e-mail address.
Please Note: If you choose not to receive My Oral Health E-mails, you can opt-out by changing your communication preferences via the website or click the “Unsubscribe” link on the bottom of each My Oral Health E-mail.
Your dental plan reserves the right to revise or otherwise update the terms and conditions of this Consent.
